Instructions for SCJAS paperwork

Online Registration - please make sure you print a copy of the page for your records BEFORE you submit the info.
Title Page for Written and Oral Presentations - You MUST have Adobe Acrobat to use this page. If not, you may download it for free at the link provided on the main menu. This page is designed for you to type directly onto. After you have filled in all the information, please click on the print button on the bottom right corner, have it signed by the appropriate people, and then mail it in to Dr. Privett - address given on the form.
Mentored Student Self evaluation Form - if this is a mentored project, a MENTORED STUDENT EVALUATION FORM must be attached to your research paper. If you email your paper the MENTORED STUDENT EVALUATION FORM will be a separate attachment. If you send your paper by regular mail the MENTORED STUDENT EVALUATION FORM should be attached to EACH research paper under the cover sheet (no exceptions) . In addition, students must have their mentors fill out a SUPERVISING SCIENTIST FORM which must be sent DIRECTLY from the mentor to Dr. James Privett BEFORE the research paper will be entered into the competition.
Supervising Scientist Form - For mentored students ONLY. This form is to be filled out by the student AND the mentor. It is to be signed by the mentor and mailed, along with the Mentored student self evaluation form, to Dr. James Privett.
Research Paper - to be submitted via postal mail to Dr. James Privett at USC Sumter / 200 Miller Rd. / Sumter, SC 29150-2498.
